[CHILDCARE] Special Benefits for Child Rearing Households (Temporary Allowance)

The city of Yokohama sent us the documents for Special Benefits for Child Rearing Households in the mid-July and you must have received this orange envelop if you or your partner is eligible to receive the benefits. To our regret, there are seems to be no English version, so we'll try to translate what is written on these documents.

There are two sheets of paper and an envelop. This time you DON'T have to put a postal stamp to send it back. The green one is a description of the benefits, and the gray one is a form to fill in and to be send back.



A: Special Benefits for Child Rearing Households of the city of Yokohama
It is to supply child rearing households with temporary allowance to reduce (financial) effects on them and to encourage their consumption after the tax raise (to 8%) in April, 2014.

B: [Target people] Those who fulfill all the following conditions:
1) Those who were registered as resident in the city of Yokohama on January 1st, 2014 (we call the day "base day")
2) Those who were the recipients of child allowance or special alliance for the month of January 2014.
3) Those whose income in the year 2013 did not exceed the limit of child allowance. 
*Those who have been taking care of a target child and became a new recipient of his/her child allowance after the original recipient passed away after the base day (the spouse of the original recipient, for example) are eligible for the benefits.

C: [Target Children] Those who fulfill all the following conditions:
1) Those who were targets of child allowance for the month of January 2014. *1 *2
2) Those who are not targets of Temporary Welfare Benefits.
3) Those who are not taken care of the recipient of public assistance. *3
*1 Those who were born on the base day or who moved in from a foreign country on the base day and were targets of the child allowance for the month of February 2014 are included.
*2 Those who were third-graders of junior high school on the base day are targets even if they already graduated from junior high school when the application is turned in and the benefits are supplied. 
*3 Those whose assistance was stopped on the base day or it was stopped or abandoned during January 2nd to March 31st 2014 are excluded.

@Exception even if the children fulfill the conditions above:
-Those who passed away between the base day to the day when the supply of the benefits is decided.
-Those who exceed the period of stay on the day when the supply of the benefits is decided.

F: [How to Supply] We will transfer the benefits to your bank account. 
-To avoid errors, the city of Yokohama ask you to use your bank account where you receive (regular) child allowance.
-It will take one or two months to receive the benefits after you turn in the application form.

G: [How to Apply] By Mail (Please use the enclosed envelop to return)

H: [Application Deadline] Postmarked by January 16th, 2015
-Special benefits for child rearing households will not be supplied if the application is not done by the deadline.

K: Please watch out for billing fraud or fraud of personal information relating to Special Benefits for Child Rearing Households.
-The local governments and the MHLW will NEVER ask you to use an ATM (automated teller machines of banks or convenience stores).
-You can Never have someone transfer money to your account by controlling an ATM by yourself.
-The local governments and the MHLW will NEVER ask you to pay any charge for the Special Benefits for Child Rearing Households.
-The local governments and the MHLW will NEVER ask your personal information such as the number of your bank account on the phone.
-Please report your local police or the Exclusive Police Consulting Line (#9110) if you get any phone call or mail to your home or workplace by someone who call him/herself an officer of the local government or the MHLW.


1: Please read the Agreement (m) well, fill in the form and turn it in.

2: Write the date when you fill in this form.
(2014 = (Heisei) 26, 2015 = (Heisei) 27), then month number (January = 1) and day.

3: Write your (recipient's) name (a) and birthdate (b). * Only the recipient of child allowance for the month of January 2014 can apply.
a: the 印 mark is where you stamp your name, but you don't need the stamp by signing by yourself.
b: The model person was born on July 5th, 1975. You circle 昭和 in the middle if you were born before January 7th, 1989 and circle 平成 for those who were born after.
4: Write your address on January 1st, 2014.
5: -The target children are those who receive (or were to receive)   child allowance for the month of January 2014. Those who applied child allowance in January 2014 and became the target in February are out of the target this time.
-The names, relationship, birthdate of the children and the classification that if the recipient and the children live together are printed (as of January 2014).
-Write the address of the child if living separately (d) .
-Regarding the tax law, circle 該当 (applicable) if the applicant takes care of the child, and circle 非該当 (not applicable) if not (c) .

6: If there are any children 非該当 (not applicable, those who are not taken care of by the applicant) in the list of children above, write the child number (e) , the name of the person who take care of the child (f), birthdate, relationship (g, 父 Father, 母 Mother, 祖父 Grandfather, 祖母 Grandmother etc.) and check if the person was imposed the tax in 2014 (課税 Tax Imposed 非課税 Tax Not imposed).
*If any child in the list was taken care of by a person who is not imposed the tax, the child is not the target of Special Benefits for Child Rearing Households. In this case, s/he may be the target of Temporary Welfare Benefits so please confirm your condition.

7: -The target of this allowance is the children who receive (or were to receive) child allowance for the month of January 2014.
-Those who applied child allowance in January 2014 and became the target in February are not included in this number.
-Those who were born on the base day or who moved in from a foreign country on the base day and were target of the child allowance for the month of February 2014 are included.
i: The number of target children
j: Amount of benefits to apply (*the number of target children * 10,000 yen)

k: Bank account for transfer
To avoid errors, the city of Yokohama ask you to use your bank account where you receive (regular) child allowance.
If you use the bank account which is used for child allowance, it is not necessary to attach any documents to confirm your account.
8: If you name the account where you receive child allowance, please check in the box.
[] I wish the benefits to be transferred to the bank account for child allowance.

9: If you wish to name another bank account, check in the box (l)  and attach the photocopy of the bankbook and your ID. You cannot receive the benefits if you don't attach these documents.
-The model of the bankbook
Please enclose the copy of the page as in the image where the branch code, the account number, the name of the owner of the account, the bank code, name of the bank, name of the branch and the bank stamp (which are red-squared in the image) are printed.
-If you use Yucho Ginko (JP Bank), enclose the copy of the page where the three-digit number of the branch and seven-digit account number are printed.
-You cannot use the account of your spouse or child.
-For your ID: driver's license, passport or resident card can be used.
*If you name another account, it will take more time for the money to be transferred than when you use the account for child allowance.

m:  Agreement of the applicant and the caretaker of the children
(I agree with:)
1) To judge the eligibility of Special Benefits for Child Rearing Households, that the city of Yokohama will use the public notes such as the tax information that the city owns or the city will inquire other administrative organs for necessary information of the applicant and the caretaker of the children as in the 2 on the left page.
2) Turning in necessary documents (for the city) to confirm the eligibility for Special Benefits for Child Rearing Households.
3) Dealing with this form as a bill after the city decide to supply the applicant/caretaker with Special Benefits for Child Rearing Households.
4) That the city will regard the application is to be withdrawn when the transfer of Special Benefits for Child Rearing Households to the applicant is not completed because of errors and the errors can not be solved by February 27th 2015.
5) That if it turns out that the applicant is not eligible to Special Benefits for Child Rearing Households after it is supplied, it must be returned.

10: Example cases of "be not eligible"
(1) It turns out that the applicant is the target of Temporary Welfare Benefits after the allowance is supplied.
(2) There is a lapse or reduction in child allowance dated back to the month of January 2014.
(3) The income of the applicant exceeds the limit after the final tax return etc.
